About this service

The drastic shift in expectations, responsibility, and overall environment between primary school and high school can often lead to adjustment difficulties for young people. This difficulty is further exacerbated for those who already struggle with emotional regulation, academic achievement, social engagement, and adverse life experiences.

This service is aimed at addressing these difficulties by working with the individual and their family to ensure a smoother transition from Swallowcliffe Primary School to Playford International College. The service offers therapeutic support and case management to best achieve desired outcomes.

Support will predominantly take place on school grounds, however depending on the needs of the young person and the family, other arrangements can be made.

Eligibility

Students preparing to graduate from Swallowcliffe Primary School with the intent of attending Playford International College the following year. Referrals can be made by the eligible young person, as well as any stakeholders that have an interaction with a young person that matches eligibility criteria.

11-13 (years 6-7)

Hours

Monday to Friday, 9:00am – 5:00pm.
